Disclosure and Waiver of Liability Agreement

I, the undersigned (“Client”), acknowledge that I have read, understood, and voluntarily agree to the terms of this Disclosure and Waiver of Liability Agreement.

TwiggyFitness, Christina Davis, is a certified health coach specializing in nutrition. She is not a physician and does not diagnose, treat, cure, or prevent disease. Her services are not a substitute for medical care provided by a licensed physician or other qualified health care professional. No representations, claims, or guarantees are made regarding the effectiveness of any recommendations or services provided. Recommendations are based on a combination of education, coaching experience, and knowledge of natural health literature.

I understand that the nature of the recommendations and services provided will be explained to me, and that I will have the opportunity to ask questions. I understand that I am not required to accept any recommendation or service.

Individualized recommendations are provided for educational and informational purposes only. Any action I take based on the consultation is at my sole discretion and risk. I am strongly encouraged to maintain a relationship with one or more licensed physicians or other appropriate health care providers for any medical concerns. For example, if I have children, I should consult a pediatrician; if I have cardiovascular disease, I should consult a cardiologist; and if I have cancer, I should consult an oncologist.

I understand that I have not been instructed to discontinue any treatment, medication, or care from any other medical specialist or health care provider unless specifically directed by that provider. My signature confirms that I am providing this consent before receiving any service, advice, or recommendation.

Emergency Disclaimer: If I experience chest pain, difficulty breathing, severe symptoms, or a medical emergency, I understand that I should call 911 or seek immediate medical attention.

Financial Policy: Client is fully responsible for all professional services received. TwiggyFitness does not contract with insurance companies and does not bill insurance for services. I understand that I am responsible for all charges.

By signing below, I agree to comply with the policies above and acknowledge that I understand the terms of this agreement. Client agrees not to hold the Company liable for any costs or damages related to the services provided, except in cases of willful misconduct or gross negligence. I understand this consent agreement and execute it freely and voluntarily.
